Reporting to the Sr. Director, the Community Engagement Specialist works to promote Careforth’s Structured Family Caregiving program, building relationships with local Care Coordinators, Area Agencies on Aging and other organizations/partners, and to generate interest in and referrals for Structured Family Caregiving. Additionally, the Community Engagement Specialist works locally, in collaboration with care coaches and other professionals, to support family caregivers who are caring for their loved ones at home. The Community Engagement Specialist visits families at home, conducts family-centered assessments, and identifies the support a caregiver needs to help provide care at home. This individual partners with a centralized coaching team to ensure ongoing engagement, support, intervention as needed, and collaboration with Care Coordinators.

Reporting to the Manager, Care Management, the Supervisor, Care Management will supervise the day-to-day operations of a team of Care managers (RN and non-RN) whom provide case management services across the continuum of care (assessment, care planning, care coordination, and monitoring and evaluating services). Under the direction of the Manager, Care Management this individual will assist with creating action plans in support of quality improvement efforts and positive health outcomes, with a focus on building person-centered plans that fosters continuous improvement and caregiver engagement.
Reporting to the Manager, Enrollment, the Enrollment Specialist is responsible for ensuring all required paperwork and authorizations are received in a timely manner in order to efficiently move care recipients into service. This individual will work directly with the Manager, Enrollment and Assessment Teams in an assigned territory to optimize the customer experience for new families coming into the program. This will include creating efficient processes that relieve the administrative burden of referral partners and decrease the timelines for accessing company services. This individual will drive process improvements in cross functional operations including referral intake, background verification, and securing of initial authorizations. This role will also manage relationships with referral sources and PCP offices to enable efficient communication. In addition, they will oversee administrative efforts in an assigned territory.
The Manager, Contact Center will report to the Director, Contact Center and will manage and oversee teams within the Contact Center responsible for managing telephonic and virtual referrals for all Careforth programs. This individual will manage a team responsible for the initial and on-going enrollment of care recipients and caregivers into Careforth Programs.
